We focus on keeping you healthy with services such as:

• Four 360 Neighborhood Wellness Centers that offer high-quality Primary
and mental health care.

• Vital housing services, including Street Outreach, Street Medicine, Emergency
Shelter, Rapid Rehousing Medical Respite, and permanent supportive housing.

• PATH program that supports homelessness into homes solutions.

• Intensive case management, behavioral health services, substance abuse support, and other therapeutic services for children and adults with intellectual or developmental disabilities (IDD).
